Please complete this application by providing your personal details, including verification of your age and retirement status. This information will help us ensure your eligibility and provide you with access to tailored opportunities and resources.

Proof of Age (65+) or retired:
As part of your application, you are required to submit proof of your age. Acceptable forms of proof include:

  • A valid government-issued photo ID (e.g., driver’s license, passport, or state ID)
  • Birth certificate
  • Any official document clearly stating your date of birth

You will have the option to upload these documents as part of the application process.

By submitting this form, you agree to the following:

Membership Agreement:

  • As a Las Reinas Member of NLBWA-IE, you agree to participate in programs, events, and opportunities that support the organization’s mission to empower Latina entrepreneurs.
  • Your membership is valid for one year from the date of approval and is non-refundable.
  • Membership benefits are personal and non-transferable.

Proof of Eligibility (Age 65+): You agree to provide verification of your age (65 years or older) as part of your application. Acceptable documents for proof of age include:

  • A government-issued photo ID (e.g., driver’s license, passport, or state ID)
  • Birth certificate
  • Any official document clearly stating your date of birth

Failure to provide valid proof of eligibility may result in a delay or denial of your membership application.

Liability:

  • NLBWA-IE is not liable for any outcomes related to your participation in programs, events, or connections made through the organization.
  • Participation in NLBWA-IE activities is at your discretion and risk.
